Using Imedic RPM devices facilitates CCM in improving patient health care. By analyzing the patient’s health data generated by Imedic devices allows caregivers to provide reactive care. Incorporating RPM helps in extending chronic care management and in improving patient’s quality of life. Since U.S. Centers for Medicare & Medicaid Services (CMS) recognizes the two services CCM and RPM complementary billing to the patient under this program can be done in the same month.

CCM is provided to patients who have two or more chronic conditions expected to last at least 12 calendar months or patients with acute exacerbation/ decompensation, or functional decline, having chronic conditions which place them at significant risk of death or when comprehensive care was monitored, revised, implemented or established. Imedic RPM devices open new doors for providers to provide services by adopting telehealth and RPM technologies. Imedic is paving a path for providers to embrace new technologies and services considering CCM has complex requirements.
