RPM Billing Codes

Code 99453 includes reporting of the work involved with familiarizing the patient to the RPM process, the initial device set-up, and patient instruction with training for each episode of care. A physician or other qualified health care professional orders an FDA-defined remote patient monitoring (RPM) device that is given to a patient, and which is used for the purpose of monitoring, collecting, and reporting health-related data to the provider, and not only limited to pulse oximetry, blood pressure or weight.

Code 99454 is used to report each 30 days of device recording and transmission. When the duration of monitoring is less than 16 days for a 30-day period, this code should not be reported. Include reporting this code for supplying the device and for treatment with a device approved by FDA. These codes are used to report treatment management of a patient currently monitored remotely for physiological parameters.

Report 99457 once per calendar month when at least 20 minutes of live interactive communication is provided, regardless of the number of modalities performed. These codes may be reported simultaneously with chronic care management, transitional care management, and behavioral health integration services. The documentation and reporting for the time involved in performing this service should be recorded separately and it should be distinct from other services also would not count towards the required time for both services in that single month. There has to be a live and interactive communication with the patient and/or caregiver, which is a requirement for this code to be reported.

CPT code 99457 is reported for the first 20 minutes of clinician time per calendar month, and CPT code 99458 for each additional 20 minutes of the clinician with the patient. Use 99457-99458 only when another more specific code is not available. Codes 99457 and 99458 may be reported with 99439, 99487, 99489, 99490, 99491, 99495, 99496, 99484, and 99492-99494, but may not be performed concurrently. Do not report 99457 or 99458 with 99202-99205, 99211-99215, 99221-99223, 99231-99233, 99251-99255, 99324-99328, 99334-99337, or 99341-99350. Do not report 99457 with 93264, 99091, or in the same month as 99473-99474.

CPT code 99091 should be reported no more than once in a 30-day period to include the physician or other qualified healthcare professional. With the technology used in Imedic devices, it is possible to gather the health data from a patient from any location and transmit the same data to the healthcare provider. The data is used for review by the healthcare provider and helps in the treatment, recommendations especially to patients with ongoing and/or chronic disease.
